The first is where the feature you worry about is something other people might notice but don’t think of as being important or worth worrying about. The second is where the feature you worry about is something other people simply don’t see; this is sometimes called an imaginary defect.
Friends and relatives often try to help people with BDD by saying the problem is not as bad as the person thinks, but this usually doesn’t work very well and may even make things worse. When you have BDD, the defect seems very real and very important to you so when others say it is not a problem you probably don’t believe them.
